We're thrilled that you want to work with our exceptional staff to assist in your child's development.  There are a few steps you'll need to take before we can get started.

 

1. Contact us to check for availability by calling 678-424-8267 or sending an email to dreamworks@dmyf.info
   1. We provide clinic-based services weekday afternoons and Saturdays
   2. We are currently in-network with Blue Cross Blue Shield and Amerigroup
2. Obtain doctor’s prescription
   1. Stating: “Occupational Therapy (and/or Physical Therapy) evaluation & treatment”
   2. Include child’s date of birth, diagnosis, & diagnosis code
   3. Must be renewed every 6 months
3. Download, complete, & return registration forms:
   1. Welcome letter
   2. Registration letter
   3. Notice of privacy notice
   4. Registration form
4. We will contact you to schedule an appointment for your child’s initial evaluation
